Physiotherapy Self-Referral

Physiotherapy Self-Referral

Musculoskeletal (MSK) physiotherapy involves the assessment and treatment of muscles, tendons, ligaments, bones, joints, nerves and other structures in order to:

  • improve your movement and strength
  • help you to do more of your normal activities
  • help you to understand and manage your condition.

We do not recommend self-referral if:

  • You feel acutely unwell, including unexplained weight-loss, persistent cough, or current fever
  • Visual disturbance
  • Severe constant headache
  • Nausea or vomiting
  • Problems with speech or swallowing
  • Dizziness and blackouts on movement

We are unable to accept a self-referral if:

  • you are under the age of 16 (under the age of 18 require an adult present at all appointments)
  • you require a home visit. Please ask your GP to refer you to Domiciliary Physiotherapy Services.
  • your main symptoms are related to an underlying diagnosed neurological condition (MS, Stroke, Parkinsons etc)
  • you are seeking treatment for a respiratory condition (for example, COPD)

Once completed you will be registered to our systems and asked to complete a short questionnaire regarding your symptoms and expectations.

Please note: If you are under 16 you are not suitable for our service.
If you don’t know or are unsure of your NHS number you can get it on this website
